Position Summary: As a Community Care Worker, you will support older people to live safely and independently in their own homes—while building genuine, ongoing relationships that make a real difference to their daily lives.
In this role, you will work with across a group of clients, providing personalised care and support tailored to their individual needs, preferences, and routines. You will also have the opportunity to deliver continuity of care—getting to know your clients over time and supporting their wellbeing, independence, and sense of connection.
Your work will include a mix of practical support and meaningful engagement, helping clients not only with day-to-day activities, but also to maintain their confidence, dignity, and quality of life.
We are committed to providing structured, reliable rosters and thoughtfully planned schedules, allowing you to focus on delivering high-quality care while building rewarding, lasting relationships with the people you support.
Key Responsibilities:
* Provide personal care and support to clients, including assistance with daily living activities such as bathing, dressing, and meal preparation.
* Support clients with medication management, ensuring that they follow their prescribed treatment plans.
* Assist with mobility and transfers and provide companionship to enhance clients' emotional well-being.
* Develop and maintain strong, trusting relationships with clients and their families.
* Collaborate with healthcare professionals and other service providers to ensure coordinated care.
* Document and report changes in clients' conditions and any concerns to the appropriate parties.
* Adhere to all safety, hygiene, and privacy protocols.
Qualifications:
* Certificate III Individual Support, Aged Care, Disability or equivalent e.g 2nd year Bachelor or Nursing
* Experience working in aged care or community nursing is essential
* Current First Aid and CPR certificates must be held at the time of application.
* National Police Clearance must be held at the time of application.
* The right to work in Australia
* Previous experience in a caregiving or healthcare role is advantageous.
* Strong interpersonal and communication skills.
* Compassionate, patient, and respectful approach to care.
* Ability to work independently and as part of a team.
* Reliable transportation, Comprehensive car insurance and valid driver's license.
