Job Description
We are seeking a highly skilled and motivated Primary Care Mental Health Practitioner to join our team. The successful candidate will be responsible for providing mental health support to Primary Care Networks in Knowsley.
The role involves working primarily in primary care, but also across the interface between Primary Care and Mental Health Services to provide advice, support and assessment to Local Primary Care Teams, Service Users and Carers.
The post holder will see new presentations of primary care mental health problems and work to improve the experience for patients with mental health needs who would otherwise have seen a GP.
Main Duties
* Provide a safe and effective day-to-day operation of the service, including the handling of clinical enquiries.
* Undertake telephone triage screening assessments to assess the level of risk/priority with each patient.
* Work with patients to support adherence to prescribed treatments, monitoring mental health medication and considering interactions with physical needs.
* Manage individual case loads, providing short-term interventions and support under the guidance of the GP.
* Collate and organise contemporaneous information about opportunities and resources in the local area that may benefit clients referred to the service.
* Evaluate outcomes and maintain accurate clinical data.
Required Skills and Qualifications
* Appropriate professional qualification or experience at Degree Level e.g. RMN, AHP, Social Worker.
* Evidence of post-registration study/continuing professional development e.g. Mentorship in Practice.
* Working towards master-level education.
* Leadership and management skills, with experience of managing cases and case loads, supervising staff and working autonomously across organisations/boundaries and with other agencies.

Person Specification
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to lead change management and contribute to developing care excellence within the ward/team area.
They should also demonstrate knowledge of relevant mental health legislation, evidence of leadership and management skills, and experience of working with a range of people with mental health problems in various healthcare settings.
A demonstrated understanding of Equality and Human Rights is essential.
Additionally, the successful candidate will be able to build purposeful relationships with internal and external stakeholders, possess excellent listening and negotiation skills, and have a proven track record of delivering high-quality services.
