Position Overview
Stream Healthcare is recruiting a Support Coordinator (Homelessness) for Mount Isa. You will be working with families and carers, building relationships and working within a tight-knit community. Be a part of a vibrant and extremely supportive team and an organisation that will invest in your growth This role will see you assist with bettering the outcomes for all clients. Responsibilities
Assist with improving outcomes for all clients and support coordination for homelessness. Build relationships with families and carers within the community.
Selection Criteria
* Relevant qualification with relevant experience within Support Coordination
* A minimum of 2 years demonstrated experience working in the community services sector or related
* Intermediate computer skills, data entry, database experience and sound general administration skills
* Leadership experience within NDIS employment services
