Overview
Allied Health Clinician Level 3 – Transdisciplinary Physiotherapist – Quick Access Response Team (QuART) at Wollongong Hospital. QuART is a dynamic, interdisciplinary allied health team providing care coordination and services to patients in their homes to prevent admissions or support earlier discharge from hospital.
Responsibilities
* Provide specialised clinical services to patients across the Illawarra Shoalhaven Local Health District, including high-quality outpatient and/or inpatient services for referred patients/clients/consumers.
* Act as a consultant within the allied health disciplines (Nutrition & Dietetics, Occupational Therapy, Physiotherapy, Speech Pathology) and lead your discipline within an interdisciplinary team.
* Deliver coordinated, at-home allied health support and interventions for 2 weeks per patient to help avoid admission or support discharge.
* Collaborate with Emergency Department clinicians, inpatient teams, outpatient and community services to identify suitable patients and coordinate ongoing care with ISLHD outpatient/community services and GP networks.
* Liaise with external agencies and services to coordinate follow-up care and prevent hospital representations.
* Maintain and support service delivery across Wollongong and Shoalhaven hospitals, with some cross-site support as needed; primarily community-based visits to patients at home and hospital-based discharge facilitation.
Qualifications and Requirements
* Applicable vaccination requirements for a Category A role.
* Working With Children Check (WWCC).
* Current AHPRA registration or eligibility for membership of the professional association for disciplines not requiring AHPRA registration.
* Valid NSW Driver's Licence.

Benefits
* Salary: $56.69 - $58.52 per hour + 12% super and salary packaging
