Ready to step up into a Care Coordinator role?
We are seeking an experienced Registered Nurse to join our Stanthorpe Aged Care Service as a Care Coordinator, playing a key role in coordinating high-quality, person-centred clinical care for our residents.
In this pivotal position, you will provide clinical leadership and oversight for residents with complex needs, ensuring care is safe, coordinated, and aligned with our Positive Wellbeing Model. You will work closely with the Service Manager and clinical team to support service quality, compliance, and continuous improvement.
About the Role
As Care Coordinator, you will:
* Coordinate and oversee baseline and focused clinical assessments
* Develop, review, and monitor individualised care plans
* Ensure accurate clinical documentation and AN-ACC data collection
* Support clinical programs, audits, and quality initiatives
* Liaise with external service providers to ensure seamless care delivery
* Provide guidance and leadership to clinical staff, promoting a culture of safety and accountability
About You
You are a confident clinician ready to take the next step, with a strong focus on care quality, leadership, and resident wellbeing.
Essential:
* Registered Nurse (AHPRA registered)
* Minimum 3 years’ post-graduate experience (aged care or acute setting)
* Strong leadership, communication, and decision-making skills
* Competent with clinical systems and Microsoft Office
* Current CPR & First Aid
* Ability to obtain required police and insolvency checks
* Full Australian working rights (no sponsorship available)
Desirable:
* Postgraduate qualification in Gerontology, Dementia, or similar
* Experience using mobile clinical documentation systems
