Job Description
We are seeking a highly skilled Support Coordinator to join our team. The successful candidate will work directly with participants to implement their NDIS plans, coordinate formal and informal supports, and ensure services align with the goals and preferences of each individual.
The role focuses on delivering high-quality, person-centered support coordination to NDIS participants, including those with complex needs. The position is hands-on and does not involve supervision or management of other staff.
A strong working knowledge of NDIS legislation, stakeholder engagement, and compliance reporting is required.

Key Responsibilities
* Coordinate and implement NDIS plans, supporting participants to achieve their goals and exercise choice and control.
* Assist participants in managing their plan budgets, including interpreting service categories and tracking utilisation.
* Navigate legislative reporting obligations and ensure compliance with the NDIS Quality and Safeguarding Framework.
* Engage with complex participants and collaborate with multidisciplinary stakeholders including justice liaisons, cultural liaison workers, allied health, and community service providers.
* Support participants subject to QCAT matters
* Engage with the Office of the Public Guardian and the Public Trustee Queensland in relation to participant decision-making, financial management, or legal matters
* Advocate for participant rights and promote supported decision-making and the dignity of risk.
* Maintain accurate case notes, participant files, and progress reports in line with legislative and organisational standards.

Requirements
* 2-4 years' experience in NDIS Support Coordination service delivery
* Tertiary qualification in Human Services, Social Work, or related field (determines wage level under SCHADS Award)
* Strong understanding of the NDIS framework, participant budget management, and service coordination requirements.
* Demonstrated experience supporting participants with complex needs, including those involved with statutory bodies.
* Understanding of legislative requirements related to QCAT
* Experience engaging with the Office of the Public Guardian and the Public Trustee Queensland in relation to participant decision-making, financial management, or legal matters
* High-level communication and interpersonal skills to effectively liaise with a wide range of stakeholders.
* Competent in the use of Brevity Care Software or similar client and case management systems.
