Role Description:
* In-Home Assessments: Conduct comprehensive aged care assessments in clients' homes, identifying individual needs and goals.
* Ongoing Reviews: Monitor and review client circumstances to ensure care plans remain relevant and effective.
* Complete assessments and support plans within required timeframes, maintaining accuracy and quality.
* Connect clients with local service providers and community organisations that align with their needs.
* Collaborate with families and carers, recognising their vital role in the care journey.
* Ensure all assessments meet Department of Health standards and contractual obligations.
What you'll bring:
* Aged Care Experience: At least 1 year working in aged care, with hands-on experience in client assessments and/or service delivery.
* Relevant Qualifications: Tertiary education in a health-related field such as nursing, occupational therapy, physiotherapy, or social work.
* Professional Registration: Current unrestricted General Registration with AHPRA or an equivalent professional body.
* Comfortable navigating Word, Excel, Outlook, and client management systems.
* You're a great communicator who thrives on building trust and uncovering what older adults truly need to live well.
