Key Responsibilities:
* Deliver clinical care to clients in their homes
* Conduct comprehensive assessments and create personalized care plans
* Advocate for clients' needs and ensure their voices are heard
* Provide health education and support to clients and their families
* Monitor health outcomes and report any changes or concerns
Requirements:
* Registered Nurse qualifications with current AHPRA registration
* Minimum 12 months clinical experience in acute or aged care sector
* Current unrestricted Australian driver's licence with minimum Third-Party Damage Insurance
* Excellent written and verbal communication skills
* Intermediate level of computer skills
* Sound clinical decision-making and problem-solving skills
* Demonstrated ability to work independently and as part of a team
