Key Responsibilities
Our care partners are responsible for managing caseloads comprising individualised personal assistance and education centred around health related activities that take place outside an institution like hospitals, aged-care facilities etc.
* Caseworkers focus primarily on direct client support by providing tailored support services based on guidance imparted during assessments conducted through interviews as well routine visits periodically scheduled alongside reminders issued upon conclusion of each session when appropriate;
(i.e., periodic review) follow-up phone call following completion certain self-help measures homework assignments outlined commensurate level service required may involve collaboratively developed resource material including training materials aimed empowerment skill building pertaining broader societal issues concerned pertinent topic areas e.g.: stress management relationships domestic violence sexual assault prevention mental wellness coping strategies conflict resolution social anxiety group therapy Anger Management Parenting Skills Time Mangement Self Esteem Building Assertiveness Conflict Resolution And Other Relevant Areas Concerning Personal Growth Development Best Practice Approaches Therapeutic Techniques Such As Cognitive Behavioral Therapy Dialectical Behavior Therapy Etc...)and constant feedback -remind plan future appointments regularly assess progress if requested:
i In addition coordination between agencies providing comprehensive supportive holistic approach throughout duration assignment extended period including post-completion evaluation maintaining communication records.
Working Arrangements
The job involves working full-time schedule while being flexible so employees can personally manage their own daily routines week after week operating largely independently although also requiring ability maintain open lines regular team updates meetings designated heads departments involved coordinating various initiatives successful customer satisfaction measured.
