**About the Role and You**
You will be part of a large, supportive multidisciplinary team which provides holistic and client centred care for the clients of Monash Health Community, based at our Cranbourne and Pakenham sites. Individual assessments, reviews and care plans are core tasks. Participation in continuous quality improvement activities is an ongoing component of the program.
You will have the ability to work autonomously and as part of a team, and possess strong organisational, delegation and communication skills.
