Supporting Older Australians with Empathy and Care
This role offers the opportunity to make a meaningful difference in the lives of older individuals by providing compassionate and personalized support. As a care coordinator, you will work closely with clients and their families to deliver person-centered care that addresses their unique needs and circumstances.
You will be responsible for managing referrals, onboard new clients, and adapting care plans to meet changing needs. This may involve coordinating home visits, working collaboratively with internal stakeholders, and developing strategies to address the identified needs of Aboriginal and Torres Strait Islander clients.
The ideal candidate will possess demonstrated aged care or clinical case management experience, as well as a strong understanding of aged care funding and service delivery. Proficiency in utilizing technology, including mobile devices and computers, is also essential. Additionally, the ability to build relationships with diverse stakeholders, communicate effectively, and multitask in a fast-paced environment is crucial.
