Vivid is looking for a Support Coordinator to join our team.

If you have advanced communication and organisational skills and want to work in a value-driven organisation and great supportive team with access to salary packaging benefits, this Echuca based position could be for you.

You will be responsible for assisting NDIS participants with complex needs to build personal capacity to make decisions and choices about the implementation of all supports in their NDIS plan. The role also focuses on assisting the participant to connect with NDIS and other supports, negotiate solutions with multiple stakeholders and build capacity and resilience.

Duties include:

* Assisting NDIS participants, their families and carers, to understand, implement, manage and review their NDIS Plans.
* Coordinating supports for participants and liaising extensively with NDIA planners and a range of allied health and community-based services
* Promoting the independence of people living with a disability to achieve their life goals by assisting them to establish connections with service providers
* Regularly monitoring and reviewing plans, recording and reporting outcomes to the Participant and NDIA within required timeframes
* Enhancing participants' capacity to coordinate their own supports, including provision of information, referrals and engaging with informal/personal support networks and community-based support options

About You:

* Experience as a Support Coordinator
* Experience working with people in the Disability sector
* Working knowledge of NDIS
* Thrive in a fast-paced environment
* Excellent time management and organisation skills with the ability to work autonomously and as part of a team
* High level of computer literacy, numeracy and attention to detail
* Skill in working collaboratively with others, offering and receiving advice and support from colleagues to achieve shared objectives. Highly developed written and verbal communication, problem solving and analytical skills
