Case Manager Role Overview
We are seeking an experienced Case Manager to join our team at Australian Unity.
* The Case Manager will work closely with families, clinical teams, and external providers to deliver safe, person-centered care that enables our customers to age well at home.
* This is a full-time role based at our Ballina Branch.
Key Responsibilities
* We build trusted relationships with customers and support them to achieve their goals at home.
* We conduct onboarding assessments, develop holistic health plans, and coordinate multidisciplinary services.
* We collaborate with clinical teams, families, and external providers to deliver safe, person-centered care and optimal outcomes.
* We support customers through care transitions, including hospital discharge or changes in health needs, and follow up with offboarding where needed.
* We maintain accurate records in enterprise systems, participate in audits, and contribute to quality and continuous improvement initiatives.
* We identify opportunities for additional services and support to enhance customer outcomes and contribute to service growth.
* We promote safety, quality, compliance, and a culture of continuous improvement across all activities.
