Are you a talented Pharmacist looking to make a significant impact within regional communities? We are seeking an enthusiastic Aged Care On-site Pharmacist (ACOP). As the Aged Care On-site Pharmacist (ACOP) you will be a credentialed pharmacist who will work with MLHD's Residential Aged Care Facilities (RACF). The ACOP will bring their expertise in medication management and pharmacotherapy to provide collaborative and integrated care for all residents as part of the multidisciplinary healthcare team. They will work closely with GPs, onsite nurses, facility staff and other health care professionals to improve health outcomes of residents and build confidence with families and carers. The role of the ACOP encompasses 4 key areas: Interprofessional collaboration and communication, Medication Management, Clinical Governance and Quality Use of Medicines.

Our Ideal Candidate Will Demonstrate
A Pharmacy qualification conferred by Tertiary Educations Institutions (eg Bachelor of Pharmacy) and have completed, or working towards an APC accredited ACOP training program, thus be credentialed, and hold a valid credential number
Full and unrestricted Pharmacist Registration with AHPRA
Experience in Residential Medication Management Reviews (RMMR) and Quality Use of Medicines (QUM)
