Job Title: Aboriginal Patient Navigator
We are seeking an experienced and compassionate individual to join our team as an Aboriginal Patient Navigator.
The successful candidate will provide exceptional support to Aboriginal people, families, and communities throughout their cancer journey.
Key Responsibilities:
* Deliver culturally responsive patient navigation services in accordance with best practice guidelines.
* Cultivate strong relationships with community leaders and healthcare professionals to ensure seamless care coordination.
Requirements:
* Diploma in Community Health or a related field of study.
* A minimum of 5 years' experience working in community health, cancer services, or a related health service environment is highly regarded but not essential for this role; however you must be willing to learn the skills required. It would also help if you have developed your knowledge through formal education/training relating directly. Skills such as Diplomas/Certificates/Higher Education Qualifications which may include relevant work-related training courses gained over time - online certification remains fully accepted even if obtained from overseas institutions while specifically meeting content syllabus requirements (not generic case-by-case determinations).
