Our Children, Youth and Families team is dedicated to ensuring that every child and young person has the opportunity to grow up in a safe, nurturing and supportive environment. We work closely with Foster and Kinship Carers, families, and community partners to help children in out-of-home care thrive.
About the role
We are seeking a passionate and dedicated Practitioner - Out of Home Care to join our team on a permanent, full-time basis.
In this role, you will support children and young people to develop positive life skills, build healthy connections, and engage meaningfully in education, employment, or training. You will work closely with young people, their families, care teams, and community stakeholders to ensure they experience safety, stability, and opportunities for growth. This is an exciting opportunity for skilled practitioners who are committed to trauma-informed practice, youth work, child protection, and supporting young people with complex needs.
Responsibilities
* Develop strength-based care plans to support positive outcomes for young people.
* Assess risks and needs of children and young people in care.
* Create behavior support, crisis management, and safety plans.
* Support young people to build self-esteem, life skills, and safe connections.
* Work with staff, families, schools, and community services.
* Assist with transitions into and out of care, including accommodation and funding support.
* Keep accurate records and reports.
* Participate in community and care networks.
* Contribute to a positive team culture and continuous improvement.
* Provide on-call support and flexibility as required.
Credentials & Qualifications
* Current Blue Card.
* Current Licensed Care Service (LCS).
* Current Queensland Driver Licence.
* Degree in Health, Human Services, Behavioural Science, Social Work, Psychology, or a related field.
* Ability and willingness to travel within the service region.
* Ability and willingness to work outside standard business hours and provide on-call support.
* Completion of Hope & Healing Framework Foundation Training (10 Modules).
