Key Roles and Responsibilities
Our team is expanding, and we have a six-month fixed-term position available for an aged care assessor. This role offers flexibility and autonomy, with opportunities to work across metropolitan and regional locations, often involving local travel.
You will be responsible for conducting assessments to identify and meet the complex needs of clients, referring them to appropriate aged care services under the Aged Care Act.
This includes:
* Collaborating with clients, families, and multidisciplinary teams as required
* Working collaboratively with colleagues to promote a supportive and positive team culture
* Conducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services
* Reviewing support plans to meet changing client needs
* Working effectively both autonomously and as part of a broader team in delivering key performance indicators within identified timeframes
* Maintaining currency of knowledge relating to funded and non-funded aged care services
* Assisting with continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes
