About the Role
Join Grampians Health Ballarat’s Carer Support Service (CSS) and make a difference for carers across the Central Highlands, Central Grampians, and Wimmera regions. In this 12-month fixed-term Case Manager role, you’ll work in a supportive Mt Helen-based team, helping carers set and achieve their goals through a person-centred approach.

Tertiary qualification in Allied Health, Nursing, or Human Service

Proficient in Microsoft Office and database

Current unrestricted driver’s licenc
