About the Role

We are seeking a dedicated and passionate Registered Nurse to join our Heart Failure Team in a Temporary Part-time role. This is a rewarding opportunity to provide specialized, patient-centered care to individuals managing chronic heart failure and contribute to improving their quality of life and long-term outcomes.

As a Heart Failure Registered Nurse, you will play a key role in the delivery of care coordination, patient education, and chronic disease management for individuals living with heart failure. Working both independently and as part of a collaborative team, you will help guide patients through complex care pathways while promoting self-management and hospital avoidance strategies.


Duties and Responsibilities

* Provide comprehensive nursing assessments and evidence-based care for patients with heart failure in inpatient and outpatient settings.
* Collaborate with Cardiologists, General Practitioners, Allied Health staff, and other stakeholders to deliver integrated care.

Required Skills and Qualifications

* Current registration to practice as a Registered Nurse (Division 1) with the Australian Health Practitioner Regulation Authority (AHPRA).
* Strong commitment to contribute to own and team professional development and act as a resource person and to mentor others within the speciality where required.
* Preferred experience in clinical teaching including preceptorship and/or student facilitation.
* Demonstrated effective communication skills – written, verbal, and eMR.
* Demonstrated understanding of the issues that affect the health and well-being of clients with an acute/chronic cardiac condition and the impact of this on their families.
* Able to perform effectively as a team member by liaising and consulting with other senior roles in the service as required.
* Demonstrated ability to exercise extended autonomy by holding decision-making capabilities applicable in speciality clinic practice and service delivery.
