Nurse Practitioner - Urgent and After-Hours Service, Cygnet Family Practice
Cygnet Family Practice is seeking an enthusiastic Nurse Practitioner to join their urgent and after-hours service. The Nurse Practitioner will work autonomously and collaboratively, making high-level decisions to treat health needs in both practice settings and in patients' homes.
This service is part of an innovative initiative to deliver an integrated hospital admission avoidance program in collaboration with local general practices, pharmacies, Tasmanian Ambulance Service, Primary Health Tasmania, and the Tasmanian Health Service.
This role offers a groundbreaking opportunity requiring excellence and a dedicated commitment to establishing a legacy for nurse practitioners in autonomous and collaborative practice.
The position
Key responsibilities include:
1. Advanced clinical assessment and diagnosis
2. Prescribing medications and ordering diagnostics
3. Coordinating patient care with multidisciplinary teams
4. Responding to emergency callouts as needed
5. Applying evidence-based research to practice
Qualifications include:
* AHPRA registered Nurse Practitioner with significant clinical experience in urgent, emergency, and primary health care
* Advanced knowledge of primary health issues and management of deteriorating patients
* Understanding of clinical research and its practical application
* Willingness to train and mentor registered nurses and community paramedics
* Evidence of ongoing professional development
* Proficiency with IT applications used in primary care
* Ability to initiate, sustain, drive, and evaluate change
* Member of the Australian College Nurse Practitioners

This role is based in Cygnet, a welcoming town in the Huon Valley, just 50 minutes south of Hobart. Known for its vibrant arts scene, sailing culture, and festivals, Cygnet offers an exceptional lifestyle. The town has a range of schools (K–10) and a friendly, environmentally conscious community.
The Cygnet Urgent and After-Hours Service has been operating for over 2.5 years, funded by the Tasmanian Government to improve healthcare access for vulnerable populations. It employs a unique model combining Nurse Practitioners and Community Paramedics as part of an integrated health initiative to reduce unnecessary hospital admissions and enhance continuity of care in rural Tasmania.

30 hours per week – Thursday and Friday 10am – 8.30pm, Saturday 9am – 8.30pm
