Role Overview
This role is an opportunity to make a positive impact on the lives of individuals living with mental illness across East Arnhem Land. As a Case Manager, you will be working in a holistic and culturally safe environment to provide tailored support and guidance to clients.
The goal of this position is to empower clients by addressing their unique needs and building upon their strengths. This is achieved through case management services that are trauma-informed, strength-based, and centered around client-capacity building.
Key Responsibilities
• Provide comprehensive case management services to clients across East Arnhem Land
• Develop and implement personalized case plans that address clients' needs and goals
• Foster strong relationships with clients, community members, and other stakeholders to ensure effective communication and collaboration
• Conduct home visits, hospital visits, and other outreach activities as needed
• Maintain accurate records and reports, ensuring confidentiality and data integrity
Requirements
• Bachelor's degree in Social Work, Psychology, or a related field
• Proven experience in case management, preferably in a culturally sensitive and trauma-informed setting
• Excellent communication, interpersonal, and organizational skills
• Ability to work independently and as part of a team
• Strong analytical and problem-solving skills
• Familiarity with relevant policies, procedures, and legislation
