Role Overview
Social Worker (Level1‐2) – Permanent, full‐time; Department and location: Social Work, St Vincent's Hospital, Darlinghurst. Salary ranges from $76,394.31 to $108,986.24 plus super and salary packaging.
Responsibilities
* Monday to Friday shift with on‐call requirements, primarily in the Heart‐Lung Stream.
* Provide social work services to patients, families, and carers across Cardiology, Cardio‐Thoracic Surgery, Thoracic Medicine, Thoracic and Cardiac Rehabilitation, and Heart and Lung Transplant units.
* Conduct psychosocial and risk assessments, deliver counselling, and offer family support.
* Participate in multidisciplinary team meetings to support patient care, discharge planning, and referral coordination to community supports, accommodation, and placement services.
* Advocate for patients and carers, including assistance with NCAT Guardianship applications and NDIS/NDIA processes where required.
* Contribute to clinical operations through documentation, handovers, statistical reporting, clinical cover, on‐call duties, and resource development.
* Engage in professional development activities such as supervision, education, research, student training, quality improvement, and mandatory training compliance.
Qualifications
* Degree in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW).
* Demonstrated clinical experience in a tertiary hospital or health‐related setting.
* Strong skills in psychosocial assessment, counselling, therapeutic interventions, case management, advocacy, and complex discharge planning.
* Knowledge of crisis intervention, short‐term solution‐focused approaches, and grief and loss support.
* Ability to work effectively within multidisciplinary teams in high‐pressure environments, managing competing demands with strong negotiation and liaison capabilities.
* Willingness and ability to participate in extended hours and on‐call service as required.
