We're seeking a Registered or Enrolled Nurse to join our Southport Medical Centre, where you'll support GPs and patients through chronic condition management and preventative healthcare. You'll be part of a great, supportive team that values collaboration, culture, and patient outcomes. Part-time and full-time opportunities available.
What You'll Do
* Prepare Care Plans and Team Care Arrangements
* Support patients in managing chronic conditions and improving health behaviours
* Conduct health assessments and promote preventative care
* Collaborate with GPs and multidisciplinary teams
* Manage administrative tasks related to CCM appointments
About You
* AHPRA registered Enrolled or Registered Nurse
* Prior experience in Chronic Condition Management preferred
* Strong communication and organisational skills
* Friendly, proactive, and patient-focused approach
