About the Role
These important roles work within a broader allied health team and alongside other diabetes educators to plan and deliver clinical and non-clinical diabetes education support and services to people living in the East Kimberley region of Western Australia. Services are delivered in towns and remote communities across the region in partnership with cultural health services, WA State Health Services, and other community-based organisations.
This position is based in Kununurra. The successful applicant will deliver diabetes education with a focus on diabetes prevention and minimising diabetes-related complications, using a supported self-management approach with individuals and families across the Kimberley.
Service delivery outcomes align with organisational strategic and operational plans, grant agreements, and service funding requirements.
Job Description
Key Responsibilities
* Provide a comprehensive diabetes education service across the East Kimberley, supporting individuals affected by T1DM, T2DM, T3cDM, diabetes in pregnancy, and prediabetes. Services will include clinical care, community programs, and capacity-building initiatives with a focus on prevention and self-management.
* Use an evidence-based, person-centred approach aligned with best practice standards.
* Contribute to a collaborative, multidisciplinary team environment and participate in relevant team meetings.
* Maintain scope of practice and stay up to date with clinical knowledge through professional development.
* Promote and utilise telehealth services where appropriate.
* Complete monthly and other required reporting, including service data and outcomes.
* Claim funding rebate items (e.g. Medicare, NDIS) as applicable.
* Undertake other reasonable duties as directed by management.
Community Engagement
* Coordinate community health promotion activities and contribute to awareness campaigns, including social media and staff in-services.
* Engage with regional stakeholders to support the promotion and integration of diabetes education services.
* Support consumer-focused services and participate in relevant community meetings and groups.
Innovation and Quality
* Develop, implement, and monitor quality improvement activities.
* Identify opportunities for service enhancement and integration into primary health care.
* Apply contemporary research to improve service delivery and education programs.
* Provide informed feedback on emerging programs and initiatives.
Program Development
* Liaise with local health care providers and communities to refine and improve services.
* Support skills transfer to regional health providers to help build sustainable service models.
Selection Criteria
* Tertiary qualifications in an appropriate profession with a Postgraduate Certificate in Diabetes Education (or higher) and current registration with AHPRA (or equivalent).
* Credentialled Diabetes Educator (CDE) with ADEA or working towards this status.
* Demonstrated skills and experience in delivering culturally safe, evidence-based diabetes education to individuals and groups.
* Ability to work effectively with Aboriginal clients in a culturally sensitive manner. Prior experience in Aboriginal and Torres Strait Islander communities and/or remote settings is highly desirable.
* Proven ability to plan, deliver and evaluate diabetes services and group education programs.
* High-level interpersonal, verbal, and written communication skills with the ability to build strong relationships.
* Demonstrated ability to adhere to quality standards, workplace health and safety policies, and procedures.
* Current "C" class driver's licence and willingness to travel throughout the Kimberley, including by 4WD and light aircraft. Frequent travel is a requirement of this role.
Experience and Qualifications Required
We\'re looking for someone who is passionate about providing high-quality health care and enjoys working with clients from diverse backgrounds. Experience delivering diabetes education in varied health settings is ideal. You'll have ADEA accreditation or be working towards it. AHPRA registration (if relevant) and professional association membership are required. Experience working in rural and remote health and with Aboriginal communities will be highly regarded.
