Job Description
Community Workers are staff that are required to work both in the office and in the field. They are responsible for working with and directly supporting clients and perform an integral role in ensuring services are delivered to clients, as outlined in an individual's support plan. This role involves supporting clients to maintain their independence in a respectful, safe and dignified manner.
Main Duties
Provide and deliver individual and personalised support and resources to customers, so that they may continue to live in their homes.
Access client's needs and develop or vary care plans as and when required in consultation with the Care Manager.
Additional Service Tasks
Maintain on-going communication with the client to assess essential community services requirements, identify options and link clients with appropriate services, including medical and allied health professionals, groups or programmes.
Responsibilities include, working under the guidance and supervision of medical professionals, develop and manage customer service schedules, including service planning that aims towards improving livelihood by conducting interviews, fortnightly surveys, setting goals and uncovering issues of distress.
Providing positive role-modelling and mentoring to client, planning activities which are consistent and appropriate to build resilience, community engagement and relationships.
Monitoring and Reporting Responsibilities
Prepare client's progress report based on most recent interviews, surveys and based on extent of difficulty client faces during the reporting period.
Risk Management Responsibilities
Conduct Risk assessments for clients using the WHS risk assessment templates provided by Just Better Care and prepare client specific risk management plans wherever required.
Client Welfare Responsibilities
Assessing the welfare, housing, health and employment needs of clients and working under supervision of the immediate manager to address the issues effectively.
Working with clients ranging from low to high physical and mental health care needs some of whom may demonstrate a range of challenging behaviours.
Assisting these clients in activities relating to community support and empowerment.
Demonstrate a commitment to best practice, ensuring that all services are provided in a caring and respectful way in accordance with Just Better Care's policy and procedures.
Principal Accountabilities
The provision of individualised services to customers in accordance with their support plans.
Monitoring the wellbeing of customers and informing the Care Manager of any concerns or changes.
Ensuring that Just Better Care or customer property is kept secure against loss, theft or damage and is properly maintained at all times.
Attending and participating in staff meetings and training sessions as directed by the Company Management.
Ensuring that all responsible steps are taken to protect personal safety at work and avoid adversely affecting the health or safety of any other person at the workplace in accordance with requirements under the Work Health and Safety legislation.
Participating in performance reviews.
Participating in activities aimed at the continuous improvement of services.
Complying with Just Better Care's Vision, Values, and Purpose.
Complying with Just Better Care's Staff Handbook, including relevant policies and procedures.
Selection Criteria Essential
Diploma or Bachelor degree in Community Services or closely relevant.
Minimum 1 year of Experience as a Community Worker.
Current and clear Criminal Record Check.
Working with Children Check (where required in accordance with state based legislation).
First Aid and CPR certificate.
Ability to be contacted by either telephone or email.
Ability or willingness to assist or work with customer in daily living and personal care skills.
Be reliable & productive.
Valuing attitudes towards customers.
Ability to work as part of a team and maintain good working relationships.
Commitment to self-improvement.
Good written and verbal skills in English.
Job Salary: $70,000–$80,000 Plus Super
