We are seeking an individual to join our multidisciplinary team as a Care Partner, working closely with clients and their families to understand their needs, goals, and preferences.
The successful applicant will be responsible for developing individualized support plans that cater to client needs, coordinating access to services and supports within the healthcare system, and assisting clients with budget management and financial planning.
