Healthcare Professional Opportunity

We are seeking a skilled and experienced Nurse Care Manager to join our clinical team in Tasmania. The successful candidate will have the opportunity to make a meaningful impact on the lives of people living with chronic disease, empowering them to change their health outcomes.

The role involves delivering high-quality nursing care to clients in the community, providing clinical expertise and support, assessing and identifying deterioration, communicating results to medical teams, working collaboratively with healthcare professionals, families, and carers, and ensuring accurate documentation of care and clinical activities.

Key Responsibilities:

* Deliver person-centered nursing care to clients, including assessments, care planning, and ongoing management of preventative care
* Provide clinical expertise and support to clients with chronic conditions, promoting wellness and self-management
* Assess and identify deterioration and changes in clients' conditions, communicating results to medical teams as appropriate
* Work collaboratively with healthcare professionals, families, and carers to develop and implement individualized care plans
* Provide education, advice, support, and goal-setting assistance for clients and their families to manage health conditions and promote wellness
* Ensure accurate and timely documentation of care and clinical activities

Essential Criteria:

* Current registration as a Registered Nurse with AHPRA
* More than 2 years' experience in acute care, aged care, community nursing, or similar healthcare settings
* Demonstrated knowledge and experience in client assessment, care planning, and chronic disease management
* Strong clinical and critical thinking skills, with a focus on delivering person-centered care
* Ability to work independently, manage a caseload, and use initiative
* Demonstrated ability to maintain high clinical standards aligned with compliance, governance, and NSQHSS
* Excellent communication skills, both written and verbal, with the ability to build rapport with patients, families, and colleagues
* MS Office proficiency
* Demonstrated commitment to ongoing professional development
* Current driver's license
* Ability to travel to Northern Coast of Tasmania and surrounding areas
* A satisfactory National Police Check
* Work with Vulnerable People Card (WWVP)
