About the Role
As a Client Outcome Facilitator, you will provide holistic, end-to-end case management, advocacy and practical support to clients engaged in the AOD program. You'll walk alongside people at different stages of their journey - listening, supporting, problem-solving and helping them move toward greater independence and wellbeing.
This role is ideal for someone who enjoys variety, relationship-based work and real-world impact, while working within clear frameworks, policies and service agreements.

Key Responsibilities
* Provide holistic, end-to-end case management and client support, tailored to individual needs across the client journey
* Conduct initial and ongoing client assessments, including screening, risk assessment and safety planning where required
* Develop, implement and regularly review client-centred case management plans in line with organisational policies
* Deliver practical and emotional support that builds confidence, life skills and independence
* Provide advocacy, information and referral support, helping clients access services and strengthen community connections
* Maintain accurate documentation and professional practice, contributing to a safe, compliant and high-quality service environment
What You'll Bring
* Minimum Certificate III in Community Services or a related qualification(or AOD tertiary qualification within drug and alcohol programs)
* Experience in support work, case management or a similar community services role
* A strong understanding of client-centred practice, risk management and confidentiality
* Excellent communication, organisation and relationship-building skills
* A genuine commitment to advocacy, dignity and respect for people experiencing disadvantage
* Experience working in community services, AOD, rehabilitation or aftercare settings
* Confidence working within structured programs, policies and guidelines
* A passion for working with people
