Do you want to make a meaningful difference in the lives of people with disability? We're looking for a compassionate and reliable Disability Support Key Worker to work one‑on‑one with a customer living in Supported Independent Living (SIL), helping them build skills, increase independence, and engage safely in their community.
In this part‑time role, you will work closely with one individual customer, providing personalised, goal‑focused support that helps them build skills, improve independence, and engage safely in their community. You'll play an important and stable role in their daily routine, ensuring continuity, rapport, and high‑quality person‑centred support.
Location: East Albury
Days and Hours: Full availability across Monday to Sunday across a 24/7 rotating roster is required. 30 hours per week.
Special interests that would be beneficial for this role include the ability to prepare and cook a variety of meals, and an interest in sporting events, car racing, horse racing, dining out, and market stalls.
KeyResponsibilities:
Support Tasks
* Provide safe, high‑quality personal and health care.
* Administer medication and support manual handling/complex care.
* Assist with domestic tasks and daily living skill building.
* Follow all support plans (ISP and Behaviour Support Plan).
* Support community, social, and family connections.
* Complete shift notes and required reports (incidents, mandatory, restrictive practice).
Key Worker Tasks
* Coordinate supports across health, education, community, and daily life.
* Update Team Leader, support staff, and stakeholders.
* Complete all Key Worker reports and documentation.
* Support goal planning and attend relevant meetings.
* Develop weekly/monthly plans and coordinate appointments.
* Work collaboratively with the customer's care team and allied health providers.
Qualifications
* Certificate IV in Disability, Aged Care, Individual Support, Community Services, or a related field and/or proven relevant experience.
* Compliance ready: NDIS Worker Check, Police Check, Drivers Licence
Skills, Knowledge and Experience
* Knowledge of NDIS Practice Standards and Code of Conduct
* Experience supporting people with disability
* Ability to work collaboratively with health professionals
* Strong reporting and documentation skills
* Confident computer and systems skills
* Ability to problem‑solve and work independently
* Reliable, adaptable, with strong communication skills
* Able to follow routines, procedures and professional boundaries
