Job Summary
As the Care Coordinator, you work in partnership with the Care Designer team to schedule staff to clients, respond to client queries about care plans, budgets & rosters, educate clients and providers on rights and responsibilities under new Support At Home legislation. You will also ensure that all people systems and initiatives are executed according to Nurse Next Door's operating model, purpose of Making Lives Better, core values and brand. You will drive results through the following main duties and responsibilities:
Scheduling
Follow Nurse Next Door's 'One Best Way' policy to schedule suitable Caregivers to clients
Take into consideration staff overtime rules & break shift allowances when scheduling
Liaise with the Care Designers to ensure a good staff/client 'fit'
When possible, schedule on a recurring basis with the view to 'Admire' staff and provide regular visits/hours, with consideration to their availability
Arrange and roster third party provider services to Support At Home clients
Review Support At Home client budgets to ensure care is scheduled appropriately
Client Queries
Manage an email inbox responding to client changes to rosters & general queries relating to Care Coordination
Assist with booking Caring Consults for new potential clients (intakes)
Assist Care Designer to amend the client budgets
Handling complex conversations with clients and families
Other duties may be requested from time to time depending on business requirements
Availability
9am - 2pm (can be slightly flexible on start/finish times)
Casual with the ability for full time for the right person
Weekend on-call (rotational)
Education and Experience Requirements
Experience in a high volume and fast growth environment
Exceptional written and verbal communication skills
Intentional organisation, planning, time management and multitasking skills
Demonstrated experience in customer service
