The Role
We are seeking a motivated and compassionate Occupational Therapist (OT) to join our growing team. As an OT with 4 Hearts Homecare, you will play a key role in supporting people with disabilities and older Australians to achieve their goals, maintain independence, and live meaningful lives.
You will work closely with participants, families, and support staff to provide assessments, therapy, and recommendations that are practical, person-centred, and tailored to each individual's needs.
Key Responsibilities
 * Conduct functional capacity assessments, home environment assessments, and equipment prescriptions.
 * Develop personalised therapy plans to support independence and wellbeing.
 * Provide recommendations for assistive technology, home modifications, and therapy interventions.
 * Work collaboratively with participants, their families, and other health professionals.
 * Support NDIS participants with goal-based outcomes and provide detailed reports for NDIS funding requirements.
 * Deliver therapy in a way that is flexible, respectful, and centred around the person's lifestyle and goals.
About You
 * Tertiary qualification in Occupational Therapy.
 * Current AHPRA registration.
 * NDIS Worker Screening Check, Blue Card, and Police Check (or willingness to obtain).
 * Current driver's licence and reliable vehicle.
 * Strong communication and report-writing skills.
 * A compassionate, empathetic approach with a commitment to person-centred practice.
