The Opportunity
As Manager Complex and Integrated Care, you’ll build strategic partnerships, co-design initiatives with stakeholders, and champion person-centred, trauma-informed, and culturally safe integrated care that improve access and outcomes for diverse communities.
Key Accountabilities
- Development and delivery of Activity Work Plans, using your project management expertise to oversee performance, manage risk and evaluate outcomes across multiple, complex programs.
- Lead stakeholder engagement and facilitate the codesign of programs that promote coordinated and integrated care, including building relationships with hard-to-reach groups.
- Manage and mentor a high-performing team, fostering collaboration and capability building.
- Oversee collaborative activities related to commissioning implementation of programs across the Operations team.
- Apply our health systems knowledge to innovate, contribute to organisational strategy and continuous quality improvement.
About You
You’re a strategic leader who can mentor, coach and build a culture of knowledge sharing across teams. You thrive in complex environments and have a record of improving health equity and accessibility.
Key Capabilities
- A Degree level qualification in health, aged care or a related field, with 5+ years’ relevant experience.
- Substantial experience managing multiple, complex health related projects including risk management, monitoring, evaluating and reporting.
- Proven ability to engage stakeholders and deliver co-designed health programs that achieve targeted outcomes.
- Lived or qualified experience working with hard-to-reach groups with demonstrable skills in building effective relationships.
- A connection with the Murray PHN catchment, our values and vision.
