You will be involved in a variety of family medicine including triage, health assessments, chronic disease care, preventative care and supporting patients through their healthcare journey. Working alongside GPs, nurses and a supportive multidisciplinary team, you will play an important role in care planning, referrals, procedures, wound care and health promotion.

The Role
We are looking for a motivated and community-focused Practice Nurse (RN) to join the AWAHS team full-time for a 12-month contract, with the potential to extend.
In this role you will:
* Be the first point of support for clients, coordinating urgent GP appointments and managing waitlists with empathy and professionalism
* Work collaboratively with GPs, allied health and external providers to ensure clients receive seamless, quality care
* Provide hands‐on nursing support including wound care, diagnostic procedures, chronic disease management and health assessments
* Assist with medication management and initial emergency presentations
* Support health promotion and preventative programs across all ages, from children to Elders
What We Are Looking For
We would love to hear from you if you are:
* An AHPRA Registered Nurse with a minimum of 2 years community practice experience with strong clinical experience
* Skilled in wound management, chronic disease care and health assessments
* Experienced and confident in diagnostic procedures and minor interventions
* Accredited Nurse Immuniser (or prepared to obtain)
* Passionate about health promotion, prevention and community wellbeing
* Able to thrive in a fast‐paced, team environment where collaboration matters
* Strong interpersonal skills, integrity and excellent time management
* Understanding of health issues impacting Aboriginal people
* A current Working with Children Check and Driver's Licence
* A willingness to obtain a satisfactory Police Check
