ITACS provides a time-limited, goal-oriented service for older people who would benefit from additional low intensity therapy in their own homes, following discharge from hospital, as well as assisting with the decision-making process around long term care planning.
The social worker will maintain a case load including case management responsibilities for a number of clients from admission to discharge. The role includes comprehensive psycho-social assessments, care planning, risk assessment, education, advocacy, counselling, liaison with community agencies and relevant parties including medical teams. Support often extends to client's families and/or carers. Working in the community setting requires a level of autonomy in problem-solving without direct clinical supervision when completing in-home assessments and interventions. Ideally, this position would suit someone with at least 2 years' experience.
The position functions as part of an interdisciplinary team with a teamwork approach to the provision of home-based intervention to frail and often socially complex older clients who would like to return home but are unable to do so without additional care and support.
