Public Health Nurse - Kidney Care Specialist
Job Description:
We are seeking an experienced Public Health Nurse to fill a key role in our Kidney Care Program. The successful candidate will work closely with local health services to develop and implement strategies to improve kidney health outcomes for the community.
Key Responsibilities:
* Provide education and support to patients with chronic kidney disease
* Collaborate with healthcare providers to coordinate care and optimize treatment plans
* Develop and maintain relationships with local stakeholders to promote kidney health initiatives
Required Qualifications:
* AHPRA registration as a Registered Nurse
* Experience in chronic kidney disease management and nephrology nursing
* Ability to communicate effectively with Aboriginal communities
