About the role
Assist NDIS participants to coordinate and implement a range of support services, in line with their NDIS plan, using a person centred active support approach. You will establish strong rapport and relationships with a range of stakeholders including People with Disability and/or mental health issues, their family members, NDIS Local Area Coordinators, NDIS Planners, internal and external service providers.
This role is a permanent part time (four days per week where the days are flexible or 5 days a week with shorter days) opportunity where you will have the flexibility to work from home. There will be some travel required to meet with clients across the Central Coast so an NSW driver's licence and your own reliable vehicle is required.
