Position Details
This position supports older adults to maintain safe and accessible home environments. Guided by the Program Case Worker, the role encourages client participation in decluttering and safe‑home access strategies, enabling community supports to access the property and clients to maintain safe mobility and lifestyle.
Key Responsibilities
* Under the direction of the Case Worker, guide, support and encourage clients to declutter, maintain and clear yards for a safe and accessible living environment.
* Provide support and mentoring to individuals, carers and others as needed.
* Refer to and liaise with community supports and services to facilitate client goals around a safe environment.
* Introduce and promote health and well‑being strategies to enable self‑management of the environment.
* Provide information and facilitate connections to appropriate community supports for identified needs.
* Liaise with the Case Worker, Footprints staff and other programmes to achieve client goals.
* Attend internal and external meetings and networking opportunities when required.
* Actively adopt a culture and language of hope and optimism.
* Comply with reporting requirements and complete appropriate record‑keeping.
General
* Follow all organisational policies and procedures.
* Participate in supervision and ongoing learning and development opportunities.
* Promote and maintain a safe and healthy workplace, adhering to WHS policies and procedures.
* Work within the mission, vision, and values of Footprints.
Key Selection Criteria
Essential
* Certificate‑level qualification in ageing support, community services or a related field.
* Prior experience in a similar role with knowledge of local social groups, networks and supports for older community‑dwelling adults.
* Respectful, calm, and encouraging manner toward consumers, with patience, boundary setting and self‑awareness.
* Initiative, critical thinking and judgement based on sound reason and evidence.
* Capacity to encourage and support engagement in meaningful activities.
* Well‑developed interpersonal, verbal and written communication skills and teamwork ability.
* Computer literacy and effective client‑database management.
* Remote work capability and experience with virtual meeting platforms (e.g., Teams, Zoom).
* Current Class C driver's licence.
* Own insured vehicle.
* National NDIS Worker Screening Check or current Yellow Card.
Desirable
* Relevant training in hoarding support.
* Knowledge of local support organisations and networks.
* Current First Aid and CPR Certificate.
