We are looking for an experienced Senior Social Worker to support our Transitional Care Program (TCP) across the Perth-metro region. About the role As a Senior Social Worker, you will be responsible for undertaking comprehensive, goal-oriented and timely psychosocial assessments to understand the needs and circumstances of consumers. You will also be required to provide advocacy, case conferencing, crisis intervention, coordination of services, family support and education to enhance the consumers quality of life while living in our care. In addition to this, you will provide mentoring and guidance to our team of Social Workers, answering questions and offering support where required. You will be to work across our TCP under the support and guidance of our team of Transition Care Program Manager and Allied Health Manager. You will have the opportunity to work closely with our Multi-disciplinary Team (MDT) including Occupational Therapists, Physiotherapists, Therapy Assistants as well as the residential and home care teams. What you'll bring Tertiary qualification in Social Work (Bachelor or Master of Social Work) Australia Association of Social Work (AASW) membership (desirable) Strong communication skills with an ability to mentor and support more junior staff NDIS Worker Screening Check Influenza vaccination Full working rights in Australia Full Australian Drivers license Knowledge on SAT and ACAT processes (desirable) What we offer Our people are integral to achieving our vision of being a leading aged care provider in Australia.
