Support & Outreach Coordinator (Disability Support)

National Disability Care is seeking a reliable and proactive Support & Outreach Coordinator to deliver structured, high-quality support to a young participant in the Biggenden / North Burnett region.
While this is primarily a 1:1 disability support role, we are looking for someone who presents professionally, communicates confidently, and represents NDC positively within the local community.
About the Participant
The participant is a 7-year-old boy with Autism Spectrum Disorder (ASD) and ADHD. He requires structured, attentive support both at home and in the community.
Support focuses on:
* School pickups
* Supporting daily routines and transitions
* Building social and communication skills
* Active supervision at all times (participant can abscond and has limited danger awareness)
This role requires someone calm, observant, structured and genuinely engaged.
A male worker is preferred due to participant needs; however, female applicants are welcome to apply.
Schedule
* Tuesday, Wednesday & Thursday: 1:30 pm – 3:30 pm
* One Saturday per month: 4-hour community shift
Key Responsibilities
* Provide safe, reliable transport for school pickups and community access
* Deliver structured, skill-based support
* Maintain active supervision at all times
* Communicate professionally with the participant’s family and the NDC leadership team
* Contribute to consistent, high-quality service delivery
Requirements
* Local to Biggenden / North Burnett region
* Valid Driver’s Licence
* Own reliable vehicle with comprehensive insurance
* NDIS Worker Screening Check
* Working With Children Check (Blue Card)
* NDIS Worker Orientation Module Certificate
* Current First Aid & CPR
* Police Check
