The opportunity
Are you an experienced registered nurse ready to support clients in their own home? Join our northern team and experience firsthand the lasting impact of your work in the community. With support from your team, you will work with autonomy and flexibility, enhance your clinical skills, and build meaningful relationships with clients in the aged care community.
This full-time position is available either as a Monday to Friday role with set shifts, or as a relief role with flexible shifts across all seven days of the week.
Key Responsibilities
- Deliver exceptional person-centred care that empowers clients to live their best lives and continue thriving in the comfort of their own homes
- Be a champion for clients and their families, advocating for their rights and wellbeing
- Educate and empower clients and carers
- Own your practice - from assessments to care plans to managing complex needs
- Work autonomously while staying connected to a supportive team of nurses and health professionals
- Build strong, supportive relationships and communicate effectively with clients, families, care providers and health professionals to establish goals, provide support and education
About you
You have passion with purpose and are driven by excellence. Caring is a profession and a calling, and your work and support enriches everyone. In addition, you will have the following skills:
- A minimum of 12 months general nursing experience, with strong clinical assessment and chronic disease management skills
- Aged Care / Community experience is highly desirable
- Current AHPRA registration
- A passion for client-centred practice and collaborative care
- Robust communication, time management, and organisational skills
- A valid full Australian driver's licence
