About the Opportunity
This role involves providing assertive outreach support to rough sleepers and individuals experiencing housing crisis within a holistic multi-agency model of service.
Key Responsibilities:
* Provide place-based assertive outreach to individuals and families at risk of homelessness or experiencing homelessness.
* Support and refer clients to access relevant services addressing their individual needs.
* Collaborate with other providers in a multi-agency team to deliver a comprehensive service model.
* Assist with applications for housing and transition to longer-term accommodation options.
* Provide emotional, social, and welfare support in a culturally safe, therapeutic, and trauma-informed manner.
* Complete administrative tasks accurately and timely, including record maintenance, meeting minutes, and required reporting.
Requirements:
* Demonstrated experience delivering case management to individuals experiencing homelessness.
* Strong interpersonal and written communication skills, with the ability to build rapport quickly.
* Effective stakeholder engagement skills in a multi-disciplinary setting.
* A Certificate IV or higher qualification in a relevant area of study is highly regarded.
* Experience working with individuals exhibiting challenging behaviors and complex needs.
* The ability to work independently to achieve client and program outcomes.
Working Conditions:
* Casual employment based in Maroochydore, servicing the Sunshine Coast region.
* Flexible hours: Monday to Sunday, 7:00 AM – 11:00 PM.
* Casual rate: $46.69 per hour (inclusive of 25% loading) + superannuation.
