**Job Overview**
We are seeking a dedicated and compassionate Care Coordinator to play a pivotal role in delivering exceptional customer-centric care services.
This position entails working closely with families, conducting thorough home assessments, and crafting personalized care plans tailored to meet the unique needs of our clients.
The successful candidate will be responsible for handling high volumes of incoming inquiries, collaborating with the scheduling team to optimize service delivery, and engaging with clients to understand their requirements.
In addition to these duties, the position involves training and mentoring the care team, ensuring compliance with government regulations, and providing on-call support during evenings, weekends, and public holidays.
To excel in this role, you will possess excellent communication skills, a natural ability to build relationships, and a commitment to delivering empathetic and respectful care.
Essential qualifications include current AHPRA registration (RN/EN) or a Certificate III/IV in Individual Support, Australian National Police check, Working with Children Check, First Aid & CPR certificate, driver's license, full comprehensive vehicle insurance, and COVID-19 vaccination.
Prior experience as a scheduler, case manager, care coordinator, hospital discharge planner, or NDIS support coordinator is highly valued, along with knowledge of the Disability and/or Aged Care sectors.
Desirable skills include time management expertise, clinical wound care proficiency, relationship development abilities, and intermediate to advanced Microsoft Office skills.
