The Role

The vision for South Eastern Sydney Local Health District (SESLHD) is 'exceptional care, healthier lives'. SESLHD is committed to enabling our community to be healthy and well, and to providing the best possible compassionate care when people need it.

Plan, coordinate and deliver high quality Social Work care to patients, families and carers consistent with South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards.

The Residential Aged Care Social Worker (RACF Social Worker) works in collaboration with the ward Social Workers and Residential Aged Care Facilities (RACH) by playing an instrumental role with patients and their families/carers who are seeking transfer from hospital to a Residential Aged Care Homes (RACF). This position will work across Prince of Wales Hospital and Sydney/Sydney Eye Hospital.

The RACH Social Worker plays a fundamental role in assisting patients and their families navigate through this process. This position works closely with the ward Social Workers at Prince of Wales Hospital & Sydney/Sydney Eye Hospital in improving processes, systems and efficiency for patients who are waiting transfer to a RACF, in addition to building relationships by networking and liaising with local RACHs. The RACH Social Worker will provide services that are consistent with NSW Health and SESLHD policies, procedures and standards.

Knowledge around Aged Care and relevant hospital experience is desirable, although support and training will be offered. The successful candidate would be required to complete psycho‑social assessments, discharge planning support, and provide counselling, end of life bereavement care and therapeutic intervention to patients, their families and carers with circumstances unique to aged related issues.


Where You'll be Working

Prince of Wales Hospital and Community Health Service is a Level 6 Tertiary Referral Hospital with an inpatient bed base of 370. Each year we care for more than 70,000 patients in our Emergency Department and have around 50,000 admissions to the inpatient units. POWH offers an outpatient service and rural outreach service and provides more than 900,000 occasions of non‑admitted patient care each year, including innovative virtual models of care. We have transitioned the majority of inpatient units and wards to the Acute Services Building (ASB) including a state of the art Emergency Department and Intensive Care Unit. POWH provides an exceptional staff experience and enhanced patient and family outcomes as a result of the provision of person‑centred care.

Selection Criteria

To be considered for this position, please ensure you address the below questions as thoroughly as possible.

* Qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award providing eligibility for membership of the Australian Association of Social Workers.
* Demonstrated core competency skills in psychosocial assessments, individual and family casework, crisis intervention, counselling, advocacy and discharge planning.
* A good knowledge of community resources and experience liaising with community organisations.
* Demonstrated high level written and verbal communication, and interpersonal skills, and the ability to proactively engage with patients/clients to enhance service delivery.
* Ability to work as part of a multi‑disciplinary team and to exercise independent professional judgement on routine matters, commensurate with years of experience
* Computer skills and the capacity to use health information systems.
* Demonstrated commitment to quality improvement and evidence based practice and a commitment to ongoing clinical and professional learning
* Current drivers licence (or a willingness to travel in accordance with the demands of the position) and the ability to participate in the After Hours Crisis On‑Call service, and work across a seven day roster (including weekends).
