ABOUT THE ROLE
Are you passionate about making a difference in the lives of children and families? Gateway Health is seeking a dedicated Intake & Assessment Worker to join our Child & Family Health Program and play a vital role in supporting families to access the care they need.
As an Intake & Assessment Worker, you will be the first point of contact for families seeking support, ensuring they receive the right care and services. Your role will involve:
* Conducting intake assessments for children and families.
* Providing information, support, and referrals to appropriate services.
* Collaborating with a multidisciplinary team to ensure quality care.
* Maintaining accurate records and case notes.
* Supporting families in navigating the health system with compassion and professionalism.
* Waitlist management.

ABOUT YOU
We are looking for a compassionate, organised, and proactive professional with:
* Experience in intake, assessment, or case management (preferably in health or community services)
* Excellent, friendly communication skills to ensure families feel supported navigating services.
* Knowledge of child and family health, early childhood development, and support services
* An ability to thrive in a collaboratively team environment and to work independently.
* Relevant qualifications in community services, welfare, or a related field.
