About This Role
You will become an integral member of our dynamic Transitional Aged Care team in a picturesque riverside location. This team supports clients across the Lower Mid North Coast region within Hunter New England Health.
Key responsibilities include assessing clients and facilitating their progress towards set goals. You will receive comprehensive onsite training to ensure you are equipped to effectively manage your duties, complemented by robust support from a multidisciplinary team consisting of Registered and Enrolled Nurses, Physiotherapists, Occupational Therapists, Dietitians, and Domestic Support staff.
The TACP Care Coordinator, NUM for SAS & TACP, Aged Care Service Manager, Psychogeriatric team, Aged Care Allied Health, and administrative staff provide additional assistance.
