Clinical Assessor (Full-time, Permanent)

As a Clinical Assessor, you will have the opportunity to work independently in your region, conducting holistic assessments with a client-centred approach focusing on wellness and reablement.
Key Responsibilities:
1. Conducting holistic style assessments with a client-centred approach focusing on wellness and reablement
2. Capturing individual client information and working collaboratively to develop their support plan and unique goals
3. Supporting older Australians to continue living independently by considering supports such as assistive technology, social support services, and community care
Required Skills and Qualifications:
1. Experience in assessment and goal orientated support planning in aged, community, health or social services
2. Confident communicator with a positive approach when collaborating with clients, families, staff, and other stakeholders
3. Local knowledge and the ability to network with stakeholders within your region
4. Advanced IT skills with previous experience working across multiple platforms (Microsoft Office Suite)
5. Previous experience working with KPI's to ensure high quality client outcomes
6. Ability to manage your time effectively in an autonomous working environment
