Job Description
We are seeking a passionate, community-focused Health Promotion Officer to deliver culturally strong prevention, health education, and lifestyle programs across our sites.
* Plan, deliver and evaluate the implementation of health promotion activities.
* Support participation in healthy lifestyle programs, including walking groups, yarning sessions, nutrition, and exercise initiatives.
* Enhance pathways to GP Management Plans, Adult Health Checks, and chronic disease programs.
* Increase awareness and understanding of Closing the Gap initiatives.
* Build trusted relationships with community members.
* Promote health and wellbeing programs at events and community gatherings.
* Gather feedback to ensure programs remain community-led and responsive to needs.
* Support outreach and engagement activities to promote health and wellbeing services.
* Maintain accurate records and contribute to program reporting and evaluation.
About the Role
As a key member of our team, you will play a vital role in promoting health and wellbeing among Aboriginal communities. You will be responsible for planning, delivering, and evaluating health promotion activities that are culturally sensitive and effective.
Key Responsibilities
The successful candidate will have the following key responsibilities:
* Develop and implement health promotion strategies that address the unique needs of Aboriginal communities.
* Work closely with community members, healthcare providers, and other stakeholders to develop and deliver health promotion programs.
* Conduct evaluations and assessments to measure the effectiveness of health promotion activities.
* Identify opportunities for collaboration and partnership with other organizations to enhance health promotion efforts.
