Job Title: Transitional Care Coordinator
We are seeking a skilled and experienced Transitional Care Coordinator to join our team. This is an exciting opportunity for a motivated professional who is passionate about facilitating seamless transitions in care for consumers across the mental health division.
Key Responsibilities:
* Coordinate and oversee discharge and transition planning in line with protocols and best practices.
* Initiate discharge planning within 72 hours of admission and collaborate with inpatient and community clinicians.
* Engage consumers, families, carers, and nominated supporters to develop and implement robust transition plans.
* Facilitate multidisciplinary care and discharge planning meetings.
* Ensure timely follow-up contact with consumers within 72 hours post-discharge.
* Work closely with the Lived Experience Workforce to promote person-centred and trauma-informed care.
* Monitor outcomes, track readmission data, and contribute to quality improvement initiatives.
Requirements:
* Registered Psychiatric Nurse Grade 4 or Allied Health equivalent.
* Minimum of 3 years' experience in Acute Mental Health.
* Strong leadership, delegation, and prioritisation skills.
* Excellent communication and interpersonal skills.
* Sound knowledge of relevant legislation.
* Ability to work autonomously and within a multidisciplinary team.
* Current driver's licence.
* Previous experience as a Discharge Planning Nurse.
* Familiarity with NSQHS and National Standards for Mental Health Services.
