Seeking a skilled Emergency or Specialist General Physician to provide comprehensive physician services and medical governance for the Hospital in the Home (HiTH) model of care.
About the Role:
* The successful candidate will establish and maintain a district-wide multidisciplinary approach to care, working with Nurse Practitioners and the multidisciplinary team to deliver holistic and integrated care in line with HITH PD2025_004.
* Deliver acute and sub-acute treatments by healthcare professionals in patients' homes, residential aged care facilities, community settings or the HiTH unit.
* Lead and collaborate with multidisciplinary teams, including Nurse Practitioners, ensuring effective clinical communication using information technology and documented interpersonal skills.
Key Responsibilities:
* Develop and implement a district-wide multidisciplinary approach to care, collaborating with Nurse Practitioners and the multidisciplinary team.
* Deliver high-quality acute and sub-acute treatments in various settings, prioritizing patient safety and well-being.
* Provide leadership and collaboration with multidisciplinary teams, fostering effective clinical communication and teamwork.
Requirements:
* Eligible for registration with the Medical Board of Australia as an Emergency or Specialist General Physician.
* Demonstrated clinical leadership experience, preferably in a multidisciplinary team setting.
* Strong clinical communication skills, with expertise in information technology, written, verbal, and interpersonal abilities.
