Team Leader - Wide Bay page is loaded## Team Leader - Wide Baylocations: Hervey Bay, Queenslandtime type: Full timeposted on: Posted Yesterdayjob requisition id: JR100456**Join an Organisation dedicated to Closing the Gap in Indigenous Health**The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.The focus of IUIH Aged Care team is to work with the local Indigenous community and relevant key stakeholders to increase access to comprehensive care and provide well-coordinated, quality in home care services to eligible aged care clients.The primary purpose of this position is to in accordance with the clients' assessed needs manage the contract relationship and arrangements with the Client/Carer and review ongoing service needs, coordinate a team of care workers in the delivery of the agreed suite of services ensuring quality, timeliness and effectiveness of service delivery.
